Purpose: This Procedure establishes the minimum requirements for the lockout of energy isolation devices whenever maintenance or servicing is done on machines or equipment. It shall be used to ensure that the machine or equipment is stopped, isolated from potentially hazardous energy sources and locked out before employees perform any servicing or maintenance where the unexpected energization or start-up of the machine or equipment or release of stored energy could cause injury.
Compliance With This Program: The authorized employees are required to perform the lockout in accordance with this procedure. Failure to follow LOTO Standard Work and Procedures will result in disciplinary action up to and including termination. All employees, upon observing a machine or piece of equipment which is locked out to perform servicing or maintenance shall not attempt to start, energize, or use the machine or equipment.
1.NOTIFY: Affected personal what equipment is to be locked out.
2.SHUTDOWN: Equipment with normal shut down procedure or by the applicable “Lockout Statement” listed below

LOCK: Disconnect and lock out all energy sources and release any stored energy if possible.
a.NOTE: All personnel involved MUST have individual lock on equipment.
b.NOTE: Locks will be swapped out during shift change.
4.TAG: Ensures tag is clearly marked with required info.
5.TRY: Verify all energy sources are isolated by trying to power on equipment.
6.RETURN: Controls to the off or neutral position.

Machine Testing and Position (Maintenance/Facilities)
- CLEAR machine of tools and any other items.
- NOTIFY all affected employees that the equipment is being returned to service.
- REMOVE safety lock and restore power to energy sources.
- PERFORM testing/troubleshooting.
- RETURN to zero energy state after testing/troubleshooting is complete by following the isolation steps in this procedure.
Final Lockout Removal Procedure,
- INSPECT Work Area: Remove all nonessential items and replace all guards.
- VERIFY Employee Safety: Verify that all affected employees in the vicinity of the machine are in a safe area/position.
- NOTIFY: Affected employees notified that lockout is going to be removed
- REMOVE: Each lock/tag must be removed by individual who applied it.
- NOTIFY: Notify all authorized and affected employees that the lockout/tagout devices have been removed.
